In the quest for flawless, beautiful skin patients are turning to specialty spa treatments to exfoliate away their imperfections. Many options are available, some are similar and some quite different, but all are designed with the same purpose in mind: to remove the top layer of the epidermis and reveal a glowing, radiant complexion. In the dermabrasion-with-a-device category there is the Hydrafacial, Oxygen Facial, Hydroimpact treatment and the SilkPeel Dermalinfusion. Dermablading is a new resurfacing craze that uses a surgical scalpel to literally scrape away the epidermis.

Many consumers are turning to mild skin peels such as Obagi Blue Peel and SkinMedica’s Illuminize, Rejuvenize and Vitalize peels to brighten skin and reduce the sign of aging. Although not technically exfoliating in nature, SkinMedica’s TNS® line of products is being promoted alongside peels and other treatments to reverse wrinkles and pigmentation by helping the skin regenerate new cells.

The “not a facial” Hydrafacial combines water, suction and infusion of skin serums to exfoliate and nourish skin. Marketed as a gentler take on the traditional microdermabrasion treatment, Hydrafacial uses water instead of crystals to suction away debris from the pores while delivering targeted skin serums. When it comes to skin health oxygen is considered very beneficial. Therefore Oxygen facial treatments range from infusion with oxygen gas to oxygenated creams and serums. Then there’s the Hydroimpact, deep-cleansing ‘facial’ that uses a hydrogen water-jet system to deep-cleanse the pores and neutralize free radicals. SilkPeel Dermalinfusion introduced diamond tip microdermabrasion (no crystals) to exfoliate skin cells while delivering a liquid treatment solution. Dermaplaning (sometimes referred to as dermablading), precisely removes the top layer of skin with a 10 gauge surgical scalpel. The procedure claims to create a flawless surface by scraping off all fine facial hair or “peach fuzz.”

Halo Laser, Microneedling PRP and BroadBand Light (BBL)

The big takeaway from the proliferation of spa-grade exfoliating devices and procedures is that most of them produce only temporary results at best. Removing the top layers of the epidermis whether it’s with a scalpel, water or crystals may have an initial plumping effect but after a few days the skin always returns to its previous condition. For permanent skin rejuvenation and healing to occur the changes have to be deep down at the dermal layer. That’s why Dr. Mosser and his team prefer to offer only longer-lasting procedures such as Halo laser, Microneedling PRP and BroadBand Light (BBL) to rebuild the collagen at the layers necessary to permanently improve the skin’s texture, tone and aging.

Microneedling PRP uses an automated pen to deploy tiny needles across the skin’s surface to stimulate collagen synthesis in order to improve texture, scars and enlarged pores. The PRP (platelet rich plasma) component of the treatment is done during the same session by incorporating healing growth factors present in one’s own blood. A small amount of the patient’s blood is taken and spun in a centrifuge to isolate the platelets which are then reintroduced into the skin via the tiny punctures created by the microneedling device.

BroadBand Light (BBL) can be used in addition to Microneedling PRP or on its own to resolve vascular issues such as broken capillaries and mild rosacea as well as brown pigmentation. BBL emits a broad spectrum light energy that’s absorbed by both red pigment (hemoglobin) and brown pigment (melanin). As these pigment cells absorb light they become damaged and respond by producing healthy new skin tissue. Both treatments are better alternatives for improving the skin’s appearance over the longer-term than fleeting results from spa exfoliating procedures that only last a couple of days at best.

Vi Peel Precision + Precision Plus

Face peels have been around for many years as a way to smooth the skin and reveal a brighter and healthier complexion. The Obagi Blue Peel is an in-office medium-depth TCA facial chemical peel that’s specifically designed to penetrate gradually to minimize the downtime associated with stronger peels. SkinMedica also offers three glycolic based peels: the Illuminize Peel®, Rejuvenize Peel® and Vitalize Peel®. These peels range in strength from mild to medium and depending on the desired results are recommended for simply brightening the complexion to achieving more dramatic skin improvement.

Here at Azala we prefer to use the Vi Peel line because it produces much better results and without patients having to necessarily endure much more downtime than with Obagi or SkinMedica. Given a choice between Vi Peel and competitors’ products, it makes more sense to go with a more powerful treatment such as Vi Peel Precision (Trichloroacetic acid (TCA) and Retin-A) or the Precision Plus (with the addition of Hydroquinone and Kojic acid) to improve a multitude of skin concerns. These two highly effective professional peels can kill acne-causing bacteria to treat mild acne and also to improve acne scarring, hyperpigmentation, lines, wrinkles and texture problems.

SkinCeuticals, Obagi, and Neocutis

Medical skincare products offer patients more targeted solutions with a higher percentage of active ingredients than over the counter brands. SkinMedica’s TNS® (Tissue Nutrient Solution) is a medical line that uses a proprietary blend of growth factors, peptides, cytokines and proteins and claims 93.6% potency and impressive anti-aging results. TNS Essential Serum® and TNS Recovery Complex® are two products featured in their range. The biggest drawback with TNS® is the use of controversial ‘human’ ingredients such as fetal cells and foreskins which some people may find objectionable. There’s also the issue of efficacy which has never been supported with clinical evidence.
