Under the umbrella of non-surgical treatments for aging skin the most widely known are dermal fillers and neuromodulators. Fillers like Juvederm are used to instantly restore volume to multiple facial areas. Neuromodulators such as Botox soften lines and wrinkles by blocking the nerve impulses that cause muscle contraction. There are also longer-term options such as BBL, Halo laser, NovaThreads and SkinPen® whose goal is to minimize static wrinkles and tighten skin by stimulating collagen. Details about each type of procedure are listed below.
Dr. Mosser and his team of experienced injectors are committed to working with only the highest quality dermal fillers on the market. After having reviewed several brands, including Radiesse, Sculptra, Bellafill (previously called Artefill) and Belotero, it became clear that Juvederm® and Restylane® were the clear winners in terms of versatility, specialization and long-lasting results.
Since the introduction of their patented Vycross® technology, Juvederm® products have become the industry leaders in contouring and correcting volume loss. The Vycross® collection includes: Juvederm® Voluma, Juvederm® Vollure and Juvederm® Volbella. Selective cross-linking of hyaluronic acid molecules makes these dermal fillers more durable and with a smaller molecular weight. This allows them to be injected deeper under the skin when treating more advanced wrinkles or when a Y-Lift effect is desired. Others members of the Juvederm® family are Juvederm® Ultra and Juvederm® Ultra Plus.
The latest Restylane® range of hyaluronic acid fillers features Restylane® Defyne, Restylane® Refyne, Restylane® Silk and Restylane® Lyft. The products provide additional options when a subtle variation in texture or tissue adhesion can give patients a superior result. By far the most well-known neuromodulator on the market is BOTOX Cosmetic. It has proven time and time again to be consistent and reliable when it comes to softening fine lines and wrinkles. Furthermore, BOTOX® has been studied extensively over the last several decades. It has an excellent safety record and is FDA-approved for the highest number of medical and cosmetic indications. Even in extremely high doses used in medicine BOTOX® has been shown to be non-toxic and non-systemic.
Dysport is another neuromodulator that offers excellent safety, longevity and performance. The main difference between BOTOX® and Dysport is the botulinum proteins they contain. Both have the same active molecules, only Dysport has fewer of them and the molecules are a little smaller. These subtle differences give Dysport a greater range of diffusion, quicker onset in some patients and a high rate of biocompatibility. It’s even suitable for patients who cannot use BOTOX®. Compared with Botox, Dysport’s mechanism of action and result is exactly the same.
A third competitor called Xeomin entered the marketplace several years ago. Unfortunately many injectors reported that a higher dose of Xeomin® was necessary to produce the same results as with BOTOX® . There seems to be an issue with potency, packaging and proper dilution. For this reason Xeomin® is currently not offered at Azala.
Static lines and wrinkles (not caused by muscle movement) can be greatly improved using the Halo laser by Sciton®. It’s capable of addressing multiple skin aging concerns with one hybrid device. Halo deploys both ablative and non-ablative wavelengths simultaneously in a grid-like pattern to stimulate collagen for deep dermal rejuvenation. The treatment can also ablate or remove years of sun damage from the skin’s surface for a smoother texture and overall appearance.
The advantage of Halo fractional skin resurfacing is that it offers a high level of customization to fit any lifestyle. Excellent results in skin rejuvenation are possible with minimal downtime and discomfort.
To boost volume and skin firmness in the mid-lower face and neck, Dr. Mosser has been successfully performing NovaThreads, a non-surgical skin-tightening treatment using surgical sutures or “threads.” The threads lift and contour the face while stimulate collagen synthesis in the skin. Made out of Polydioxanone, the sutures are fully absorbed within 4-6 months but the skin-firming effects continue for 12-15 months thanks to the newly created collagen.
NovaThreads come in three varieties. Smooth Threads build up the skin’s scaffolding for a long-term collagen synthesis and tightening effect. Twist Threads have more surface area and are used to stimulate collagen and create volume in the cheeks, jowls and jawline area. Finally, Barbed Threads contain very small multi-directional barbs for anchoring and lifting loose and sagging tissue.
Younger patients seeking to dramatically smooth their complexion with as little downtime as possible may want to learn more about the SkinPen ®. The SkinPen® by Bellus Medical is a microneedling device that utilizes 12 tiny needles to create microscopic punctures in the epidermis. These micro-injuries stimulate a powerful wound healing response that promotes healthy new collagen and improves skin elasticity. Unlike the Eclipse MicroPen®, Skin Pen® goes deeper and is therefore more effective for breaking down scar tissue.
Recovery is a breeze with the SkinPen®. It’s normal for the skin to look slightly flushed, similar to a mild sunburn immediately after. This this usually resolves within 24 hours and busy individuals can resume their daily activities after a day or two. Overall, the SkinPen® procedure offers an extremely comfortable patient experience with only a topical anesthetic required for pain relief.
PRP (platelet rich plasma) can be added to microneedling to boost results even more. PRP involves using your own blood plasma to for skin rejuvenation and supporting tissue health. A small amount of blood is taken and processed in a centrifuge to isolate the growth factors which are then infused back into the skin through the tiny punctures created by the SkinPen®. As the skin heals, it starts to look smoother, tighter, more luminous and healthier overall.
BBL emits a high intensity broad spectrum visible light that targets both brown pigment and red pigment. The pigmented cells absorb the broad spectrum light, causing them to be injured and then later repaired by the body. The result is a more even and smoother complexion. A ten-year study conducted at Stanford University found that ongoing BBL treatments can reverse DNA damage. Skin cells essentially change “the expression of genes associated with longevity and aging.” This means skin cells that were treated regularly with BBL energy behaved and looked much younger than cells that were left untreated. Based on this research Sciton developed an anti-aging program called Forever Young as a maintenance regimen to slow down the visible signs of aging such as wrinkles, pigment and skin laxity.
Chemical peels such as the Vi Peel are an excellent additional to any skin rejuvenation program. They help to improve tone, texture and clarity while softening fine lines and stimulating the production of collagen. With regular use the Vi Peel can address mild skin laxity issues. They can also be combined with other skin treatments for a synergistic effect. One advantage of chemical peels is that they are suitable for darker skin types and individuals with acne.
